Taking Charge Peer Support Network: For people with HIV aged 50 and over
The next Taking Charge Peer Support Network meeting will feature an information session on Advance Care Planning, presented by Alfred Health, followed by a shared dinner. Partners and carers are warmly welcome to attend.

Advance Care Planning helps ensure your family and healthcare team understand what is important to you, and how you want to be treated if you become unable to make or communicate your own decisions. This process can include:
- Communicating: Talking with family, friends, and doctors about your values, preferences, and what you would find unacceptable in relation to your health.
- Appointing someone: Choosing a trusted person to make medical decisions on your behalf if you are unable to. This can be formalised through a legal appointment as your Medical Treatment Decision Maker.
- Putting it on paper: Recording your values and preferences in an Advance Care Directive.
Anyone can make an Advance Care Directive, but it is particularly important for those living with a chronic health condition or who are regularly admitted to hospital. Planning while your health is stable ensures your wishes are clearly understood.
